CSV Template for Adding Participants, Appointments, and 2 Misc Fields (24)

Before you fill out the CSV template, here are several important things to know:

  1. The template must be saved in CSV format.
  2. The template has 24 columns that must be present when used to import into Dialog Health.
  3. The Header Row should always be present, and must have the correct column count (24). 
  4. You can import Patients with no Appointments, just make certain ALL Appointment Fields are blank for any row with just Patient information.
  5. To obtain a copy of the template, scroll down and download the attached file.

We will now take a walk though of each of the columns from left to right on the 24 column template.  (Columns A through X)






Other things to know:
  1. To keep Messaging Campaigns accurate, It is important Appointments Cancelled and/or Rescheduled are updated in Dialog Health. 
  2. If you have two phone numbers for a patient, you can use the Additional Phone Number column to add the second number. Dialog Health will try both numbers in order to validate the persons mobile number.
  3. In order to text a person, all you need is Last Name, First Name, Mobile Number and Patient ID.
  4. It is up to your discretion if you wish to utilize Columns E through L in order to better identify patients.
  5. There are certain scenarios when you may need to use columns M through X, which are reserved for appointment or event details.If this is the case then you will want to utilize the Patient Misc or Appointment fields.

Best Practices For Naming Your CSV
When naming your CSV file, the ideal configuration would identify which center the information is coming from + a short description of the contents + data & time: 

Standard -  CenterName_ContentDescription_MMDDYY_HHMM.csv 
Examples: 
  1. MainStreetHealth_AllPatientVisits_011521_0630.csv
  2. GeneralHospital_Discharges_120221_1159.csv
  3. BroadwayCare_SpineProcedures_060621_0545.csv
